Funding Opportunity Overview

The Rural Arizona Navigator Program supports patient navigation services that improve access to coordinated, person-centered care by helping individuals overcome barriers and connect to health care and community resources.

These investments strengthen rural health systems by improving care coordination, expanding access to services, and supporting better health outcomes for rural communities.
